Please complete and submit the following Bridal Agreement once the date of your wedding is confirmed. Once one of our Bridal Coordinators receives this information, the scheduling process will begin. They will contact you to confirm that your agreement has been received and scheduled. Please be aware that this agreement is a contract that requires a credit card to reserve all appointments, however no charges will be made on your card at the time of submission. Cancellations for any and all services listed on this contract (including bridal party services) must be made 48 hours prior to the wedding day.
Outcall services - where our professionals can come to you - are available upon request and subject to availability. Please be sure to request "Outcall" on the form below when it asks which Stella Nova location you prefer. For outcall appointments, service cost will be doubled. Cancellations for services listed on this contract must be made 48 hours prior to the wedding day, otherwise your card will be charged the full balance for all services listed. A travel fee of $50 per service provider will be added to outcall services that are not within 30 miles of Stella Nova. Thank you very much for choosing Stella Nova to be part of your big day!
